Question for CORONATION HOSPITAL- Rahima Moosa Mother & Child Hospital

I am after records for a delivery of a baby male, or death certificate that took place on the 13/06/1986. I was only 17years old at the time and was 7-8months pregnant. I delivered prematurely and therefore don't have any sort of documents. I would really appreciate your assistance on this matter as i would like to have some type of documentation regarding this birth.
